HomeMeds is an evidence-based program for in-home medication management used to identify potential hazardous drug interactions, fall risks or confusion related to possible inappropriate medications, and medication side effects causing cardiovascular issues.

The HomeMeds program has been shown to cut costs and save lives. In partnership with the Elderbridge, Milestones, and Northeast Iowa Area Agencies on Aging, we have provided hundreds of community screenings since the inception of the program in 2020, empowering Iowans to comfortably and effectively discuss their medication routines with their providers to improve their quality of life.

During a screening, an inventory of the client’s medication is assessed by the HomeMeds algorithmic software. In the event an issue is detected, the client’s chart is flagged and sent to a team pharmacist for further evaluation. After review, the pharmacist reports any medication issues to both the provider and the client. Performing medication reconciliation in the client’s home environment promotes a safe and unhurried interview where the client has ready access to their medications, including over-the counter-medications and supplements.

Another advantage of the HomeMeds program is that it can be administered by community health workers, substantially increasing the number of clients served in the community.
