What Matters Most ECHO

What Matters Most ECHO is a series of short training programs intended to help build skills among those employed in nursing homes and other long-term care settings. It is based on the Age-Friendly framework, to advance person-centered care and improve the quality of life of older adults and those who care for them. The motto of this series is "What is good for the resident is also good for daily caregivers." In other words, providing What Matters Most to the residents you care for makes your life better too.

Course 1: Introduction to Age-Friendly Care

This ECHO video explains the Age-Friendly framework and introduces real-life examples to explore how Age-Friendly concepts can be used in long-term care settings.

 

 

 

 

 

 

 

 


Series Learning Objectives:

  • Describe the principles of an Age-Friendly approach to resident care, including the importance of resident-centered care, communication, the 4 Ms of Age-Friendly care, and continuity of care.
  • Develop communication skills tailored to the unique needs and preferences of older adults, including those with physical, sensory, and cognitive impairments.
  • Identify at least three strategies that can be implemented to improve workforce retention by implementing Age-Friendly care in nursing facilities.
  • Identify at least three self-care strategies that daily caregivers incorporate into their daily routines to manage stress, improve well-being, and prevent burnout.

Course 2: Communication Skills

This ECHO video shows you communication skills that you can use with residents, coworkers, and supervisors and introduces real-life examples to explore how communication skills can be used in long-term care settings.

 

 

 

 

 

 

 

 


Course 3: Dementia and Delirium

This ECHO video is about Mentation (or Mind), one of the 4Ms in Age-Friendly care. It focuses on the signs and symptoms of dementia and delirium and how they are different from one another.

 

 

 

 

 

 

 

 


Course 4: Depression

This ECHO video is about Mentation (or Mind), one of the 4Ms in Age-Friendly care. It focuses on the signs and symptoms of depression and the important roles of daily caregivers in providing Age-Friendly care to older adults who may experience depression.

 

 

 

 

 

 

 

 


Course 5: Change in Status
