Meeting Details  

Meets on the 1st Wednesday of each month (except for Jan and July). Meeting schedule is posted on our coalition HUB page and includes each month’s location.

 

Key Participants 

Quad Cities Area skilled nursing facility Administrators and Director of Nursing, along with representatives from UPH-Trinity and Genesis Post-Acute and/or Care Coordination, Healthcare systems ACO, Telligen, GWEP and other transition of care representatives, as needed.

To have a community of providers united to optimize resources that promote quality care, better health, and lower costs for all patients across the continuum of care.

 To deliver the highest quality of care for all patients in the community by improving communication among healthcare providers, care transitions, and the coordination of community care.

Our Purpose 

To create a sustainable area coalition with the focus of improving transitions of care for all patients by sharing resources and best practices, utilizing quality improvement practices, collecting data, and implementing person-centered interventions.

Accessing Hospital EMRs

See HUB page for details and tip sheets. It is your facility’s responsibility to track and keep up to date access to each hospital system’s EMRs. Failure to do so is a violation of HIPAA. Thank you for your due diligence in this.

Nursing Home guiding principles for successful partnership with Genesis and UPH-Trinity: 

    1. Respond timely to referrals. 

    1. Establish a clinical inventory of services (nursing home capabilities form). 

    1. Utilize EMR Access (Epic Care Link @ Trinity and Cerner @ Genesis) – Initial Request and Termination.  

    1. Sufficient RN staffing across all three shifts. 

    1. Encrypted protection of PHI. 

    1. Actively engage residents, patients, and responsible parties to establish meaningful advanced care plans.  

    1. Communication and/or handoff to ED before transfer of care. 

    1. Telephone follow-up with patients after discharge. 

    1. PCP warm hand-off.  

    1. Facility Administrators and Directors of Nursing to partake in monthly QC Senior Care Coalition meetings.  

    1. Communication and education to managers and clinical teams r/t coalition work.  

Health Systems guiding principles for successful partnership with Nursing Homes: 

  1. Warm hand-off before patient leaves the hospital.
  2. Send or notify of signed discharge orders as soon as complete.
  3. Discharges before noon when able.
  4. Consistent communication i.e. updates to phone lists, after-hours coverage 

Current State:

  • Data Collection to be reviewed and completed when group finds need.
  • Education to be planned by the Steering Committee which reflects the needs and interests of the Coalition. 

SMART Goals:

  1. ED Transfer rate: 1.2% (per 1,000 patients) 

  1. Readmission rate: Goal of 1.0
