Practice using the Oral Health Care Plan (OHCP) by viewing the form for Mrs. Ethel Pulikowski, who is being admitted to your nursing home. 

Mrs. Pulikowski has a complete upper denture, which is free of any markings or engraving. She has nearly all of her own bottom teeth remaining. She wears her denture all day but removes it while sleeping. Because she recently fractured her hip and has limited mobility, she needs someone to help with oral care by bringing her toothbrush and other dental products to her. Also, she often needs a reminder to brush her teeth, and to rinse and spit when she is done. However, once a reminder is given and the necessary items brought to her, she is able to clean her own teeth and dentures. Mrs. Pulikowski is under the active care of T. Ethan Gumms, D.D.S., and last visited him two months ago.

Commentary

Mrs. Pulikowski needs to have her dentures cleaned regularly and her teeth cleaned with a fluoride toothpaste.
