Specify whether there are upper or lower natural teeth present. If any broken tooth roots are present in the upper or lower, mark "Roots present."
Indicate whether the older person can clean their teeth independently or needs assistance with cleaning their teeth.
Check how often the older person's teeth need to be cleaned.
