• Check if the older person needs to be referred to a dentist, especially if there is a score of 1 or 2 for any category.
• Check if the older person and/or their family/guardian has refused dental treatment.
• Check if you are going to complete the Oral Health Care Plan (OHCP).
• Check if you are going to review the older person's oral health again and then enter the future review date.

Practice using the Oral Health Assessment Tool by selecting the appropriate score for Mr. Johnson.  Mrs. Johnson gives consent for her husband to have an examination done by the dentist.
